Rolul factorului von willebrand în boala hepatică cronică

Rezumat

Pacienții cu boală hepatică cronică avansată prezintă modificări în toate fazele hemostazei. Dovezile clinice sugerează o legătură între boala hepatică și modificările factorului von Willebrand (VWF), fiind coroborate cu date mecanistice emergente care arată un rol negativ al VWF în progresia bolii hepatice. Nivelurile ridicate de VWF accelerează formarea trombilor intrahepatici și, prin urmare, pot fi un factor determinant al progresiei bolii. Prezenta analiză se concentrează pe dovezile clinice și experimentale care susțin o legătură între funcția VWF și progresia bolilor hepatice cronice. A fost efectuată o căutare bibliografică cuprinzătoare cu utilizarea bazelor de date PubMed, Scopus și Web of Science pentru articole publicate până la data de 1 iunie 2025. Termenii de căutare au inclus următoarele combinații: „factor von Willebrand”, „boală hepatică”, „ciroză”, „tromboză venoasă portală”. Dezechilibrul VWF/ADAMTS13 corelează cu severitatea bolii hepatice și cu prognosticul slab în boala hepatică. Factorul von Willebrand este un biomarker neinvaziv promițător al disfuncției endoteliale și al progresiei bolii hepatice, dar și o țintă nouă terapeutică.
