Sintropia - boala de reflux gastroesofagian și boala cardiacă ischemică - metodelede diagnostic și tactica de tratament

Studiile descriu durerea toracică drept un simptom comun pentru asocierea bolii de reflux gastroesofagian (BRGE) și boala coronariană ischemică (BCI). În departamentul medical de urgență, peste 50% dintre pacienți cu durere toracică nu prezintă semne de ischemie coronariană, ceea ce impune necesitatea investigațiilor suplimentare. Materiale și metode. Metodologia acestui studiu a constat în cercetarea și analiza articolelor care au fost publicate în perioada 20162023, folosind bazele de date PubMed, NCIB, Medscape, Mendeley, cu utilizarea cuvintelor-cheie: „BRGE”, „boala coronariană”, „diagnostic”, „tratament”, precum și asocierea acestora. Rezultate. Ambele nosologii se manifestă clinic prin durere toracică cauzată de iritația gastrointestinală cu acid, care activează reflexul vagal și de disfuncția endotelială, care provoacă ischemie și vasoconstricție coronariană. Din perspectiva paraclinică, se caracterizează prin niveluri crescute ale IL1,6,8 și metaboliților NO implicați în patogenia bolilor. ECG și pH metria pentru 24 de ore sunt suficiente la prima etapă, pentru a determina cauza acceselor de durere toracică recurentă. IPP reprezintă prima linie de tratament pentru ameliorarea durerii toracice la acești pacienți. Concluzii. BRGE și BCI reprezintă maladii cu impact global, iar abordarea terapeutică interdisciplinară a acestor boli, culminând cu un program de tratament țintit, determină ameliorarea simptomelor și a calităţii vieţii pacientului.
